GLI Insurance is always available to assist you with your needs, especially when it comes time to make a claim! We invite you try our on-line option below:

GLI Insurance Claim Details Form (PDF)

• Click on the link above to open the PDF on-line (*Note, if you wish to save a copy to your computer hard-drive, right-click on the link above and choose "Save-As")
• Fill-out the form right on your computer. (*Note: As you work, you can save the form [.PDF file] to your own hard-drive. A saved form that is partially filled can be opened later on your computer with your previous work intact and you can continue filling it out. Completed forms can be printed for your own records.)
• Attach a copy of the completed form (.PDF file) to an eMail and send it to GLI Insurance,
• GLI will process your completed form and provide you with our professional assessment of your eligibility to make an insurance claim.
